* We close the office for lunch from 12:00 - 1:00 on Tuesday and Thursday, but work in office through lunch on Monday and Wednesday. We close the office at noon on Fridays

To make an appointment, please call (706) 235-0116. We ask you to arrive 15 minutes before your appointment to fill out your paperwork or if you choose, you can download the forms from this website, fill them in, and bring them with you to your appointment. We will need you to bring with you a valid picture ID, insurance card(s), your medications in their original contatiners, and any co-payment(s) required by your insurance.
Telephoned Medical Questions
Our medical staff are thoroughly trained to answer routine medical questions. Our nurses will answer calls at the end of the morning and afternoon. This allows the physicians to attend to his office patients with minimal distractions. Urgent problems will be handled ASAP.
We accept most insurances including: BCBS, United HealthCare, and Medicare. Please check with your insurance provider to ensure that your care is covered and your out of pocket expense is minimized.
We accept cash, personal checks, CareCredit, debit cards, and credit cards (Visa, MasterCard, Discover, and American Express)
- Our refills are done electronically, so call your pharmacy to initiate your refill request. Please try to give 1 week notice.
- Please note that our office will not refill any medications, unless the patient has been seen in the office in the last 6 months.
